Welcome Home Volunteer

Age UK South Gloucestershire

Providing short to medium term support to those who have been discharged from hospital to stay well.

Detailed Description

Age UK South Gloucestershire and Southern Brooks are both organisations who strive to ensure that people live safe and fulfilled lives, supporting people when they need it most. Volunteers will visit a client who has recently been discharged from hospital, on a regular basis to support them to meet aims set on an action plan created with their Welcome Home worker. They will encourage them to undertake tasks independently with support and guidance.

Why these skills are required

We are looking for someone who: • Has some free time to regularly visit a client • Has a non-judgemental attitude • Is friendly and flexible in their approach • Is patient and sensitive • Has good communication skills • Is empathetic, and has the understanding of the needs of older people • Can hold conversations in confidence • Ability to refer any concerns about the older person to the Welcome Home Team Coordinator

Time Commitment

Around 2 hours per week (inc travel), worked around your existing commitments.
